NP ON DEMAND provides medical evaluations and FMLA documentation services for patients located in Florida or undergoing medical procedures within the state of Florida, in accordance with applicable licensure and scope of practice.

  • Secure patient portal access

  • Continued support until the decision is made by the leave administrator.

Continuous FMLA Evaluation

Documentation Within 3 Business Days

Includes:

  • Medical evaluation via secure tele-health visit

  • Continuous FMLA documentation is completed when clinically appropriate

  • Same-day processing following completed visit

  • Autonomous Board-Certified Provider

  • Secure patient portal access

  • Continued support until the decision is made by the leave administrator.

How to Request FMLA Leave

Start Your FMLA Claim

Initiate your claim through your employer or designated leave administrator and request the required documentation. Typically, a third party leave administrator handles these requests. Your employer will give you specifics on how to start your FMLA claim.


Confirm Your Eligibility

During the claims process, ensure you meet FMLA eligibility requirements, including length of employment and hours worked. Some employers require you to be employed for one full year before becoming eligible to apply for FMLA. Checking these requirements is the employees responsibility.
